Surrogate PRE-Application

We are excited about your interest in joining us as a potential Surrogate candidate! Our team members have firsthand experience so rest assured you will be supported every step along the way. This short pre-application below will allow our staff to review your potential status.

Although we would love to accept all candidates, there are requirements for successful Surrogates:

1. Previous Birth – You must have given birth to at least one child of your own whom you are currently raising.
2. Healthy Body Mass Index (BMI) – Your BMI must be no higher than 34. Click here to cal culate your BMI.
3. Age Range – You must be between 21 and 41 years old. Note: If you’re over 38, you must have delivered a baby within the last five years and have medical clearance from your OB/GYN.
4. Healthy Pregnancy History – You should have no history of major complications during previous pregnancies or deliveries.
5. Reliable Transportation & Valid Driver’s License – You must have a valid driver’s license and access to a reliable vehicle for attending appointments.
6. Healthy, Stable Lifestyle – No Smoking, No Alcohol or substance abuse
7. Support Network – You have a good support network of family and/or close friends who affirm your decision to carry a child for another and who will provide emotional and physical support to you during your journey.
8. Partner Support and Consent – If you are in a relationship, your partner must be supportive of your surrogacy and willing to undergo a medical screening (blood test) and/or background check, if requested.
